Grade 6-12 Immunizations

Dear Parents and Guardians,

On Thursday, April 16, Fraser Health will be returning to Maple Ridge Christian School for their routine spring follow-up immunization clinic. The purpose of this clinic is to provide catch-up immunizations for any Grades 6-12 students who have incomplete immunization records, or who may have missed the clinic hosted on February 27.

How do I know if my child is up-to-date on immunizations?

Consent Forms (Sent Home Today)
Fraser Health immunization consent forms have been distributed today to any Grade 6 and Grade 9 students who may be missing immunizations. Please ask your child for this envelope. Immunization consent may be provided or declined by either:

  • returning the completed paper form to the school office by April 9, or
  • submitting consent online at immunizationconsent.gov.bc.ca/s/ (if you have not already done so).

As noted in our message on January 22, students who meet the Mature Minor criteria may provide their own consent for routine childhood immunizations. Please note a copy of this information is also this document titled School Vaccination Information for Parents.

Thursday, April 16, 9:00 AM – 12:00 PM
The Fraser Health Immunization Clinic will take place at MRCS. Students with parental- or self-consent will receive their catch-up immunizations at that time.
