Medical Treatment Not Requiring Extensive Care
I, the parent or legal guardian, hereby authorize the designated first aid attendant(s) to discern and administer appropriate medical and personal care for any injuries including but not limited to scrapes, shallow cuts, bruises, illnesses such as vertigo, nausea, or allergic reaction that the minor may experience during the Summer Sports Camp. I release any rights to be contacted for consultation, and affirm that I have provided any and all medical information concerning the attending minor’s known allergies, medical conditions, medications, and any other medical details that may impact informed decisions of the first aid attendant(s) in providing non-extensive care.

I acknowledge that no compensation will be provided for the use of such media and that all rights to the images and recordings belong to Maple Ridge Christian School.
