Completing this Application Form

Important Documents

You will need important documents on hand to complete this form such as:

• your child(ren)’s birth certificate and BC Health Card
• most recent report card
• specialist reports, current IEPs, or diagnostic testing reports (if applicable)

New family applications must include submission of the following to our office before they are considered for acceptance:

1. Application for Enrolment
2. Student Profile
3. Personal Information Parental Consent Form
4. Parent Residency Form
5. Proof of BC residency (copy of BC driver’s license)
6. Pastor’s Reference
7. Copy of birth certificate or Canadian citizenship card for student
8. Copy of birth certificate or Canadian citizenship card for parent
9. Copy of most recent report card, learning plan and/or IEP (if applicable), diagnostic testing reports (if applicable)
10. Application fee: $200 per family (non-refundable)

The information collected on this form is used and disclosed by Maple Ridge Christian School in accordance with the Personal Information Privacy Policy for Parents and Students of MRCS, a copy of which is available from the school office.

I consent to having photographs and/or work samples of my child used by Maple Ridge Christian School in the yearbook, website, MRCS app, Facebook, Instagram and other promotional material.

I understand that Maple Ridge Christian School does not publish student names on the website or social media.

Form B – Legal Residency of Parent(s) (Deceased)

To be completed and signed by the student or a knowledgeable adult (one who knew the student’s parent(s) and has knowledge of the facts respecting their decease and the matters set out in this document).

For example, at 2 months, 1 year, grade 6, etc.
